Description
This affidavit was drawn up by Grace Ford nee Atkinson the granddaughter of Sarah Ann Smith nee Hill and signed in front of a notary public on May 17, 1976. It documents the inaccuracy of the existing tombstone in Houston cemetery, as well as the dispute concerning the failure to correct the tombstone. This is also a first-hand account of the death providing a date and place of death.

"I, Grace Atkinson Ford, of Houston, Mississippi do hereby swear that I was at the bed side of my grandmother, Sarah (Sally) Ann Hill Smith when she died March 17, 1921 at Calhoun City, Mississippi.
Columbus Marble Works of Columbus Miss. promised to correct her grave stone which they erected in the Houston, Miss. cemetery, but they failed to do it.
